Job Summary:
The MDS Nurse manages the process of assessment, planning, facilitation, coordination, evaluation, and advocacy for services and options to meet the individual resident need promote quality, cost-effective outcomes while adhering to Federal and State guidelines, and in accordance with company’s policies and procedures.
ESSENTIAL JOB FUNCTIONS:
1. Manages the RAI Process from resident admission to discharge in order to maintain clinical compliance and receive appropriate funding from Medicare, Medicaid and Managed Care pay sources
2. Completes MDS and all pertinent forms required in a timely, accurate, documentation-supported and case mix optimized manner
3. Develops, with input from the interdisciplinary team members, individualized care plans reflective of the resident status
4. Coordinates resident assessments and services by other clinical departments
5. In conjunction with the Business Office Manager, provides timely information for all managed care clinical authorization and justifications
6. Manages the daily PPS/OBRA case management meetings and weekly level of care meetings
7. Other duties as assigned
Requirements:SUMMARY OF QUALIFICATIONS:
Current Registered Nurse (RN) or Licensed Vocational Nurse (LVN) in good standing and currently licensed by the State.
Minimum Data Set and Care Plan experience preferred
